At Clínica Traña we are pioneers in specialised male pelvic floor care in Costa Rica. Our physiotherapists combine extensive clinical experience, up-to-date technology and an assessment method backed by scientific evidence. This evaluation is the essential first step toward effective treatment tailored to each case.

Who is this assessment for?

This assessment is intended for people with:

  • Urinary incontinence (stress, urgency or mixed).
  • Recovery after prostatectomy.
  • Chronic pelvic or perineal pain.
  • Erectile dysfunction or difficulty with erection.
  • Premature ejaculation or pain during ejaculation.
  • A feeling of incomplete emptying or post-void dribbling.
  • Perineal heaviness, urinary urgency or sexual difficulties.

How is the assessment done?

1. Informed consent

It begins with the delivery and signing of an informed consent form that details every step of the process. Everything is done with prior authorisation, full confidentiality and the utmost respect.

2. Clinical interview

A conversation of about 20 to 30 minutes explores symptoms, surgical history (such as prostatectomy), health habits, physical activity and urinary and sexual function. This information is key to adapting care to each case.

3. Abdominal and perineal physical assessment

  • The abdomen is assessed, especially if there are surgical scars or suspected deep muscle dysfunction.
  • Muscle activation and coordination, fascial mobility and possible adhesions are reviewed.
  • Pelvic floor assessment then follows and may include:

External palpation at perineal level.

  • Internal rectal assessment (if indicated), evaluating tone, strength, reflexes and muscle synergy of key structures such as levator ani, sphincters, prostate, urethra and rectum.

Everything is done only with consent and is explained clearly before starting.

4. Functional ultrasound

At Clínica Traña we use functional ultrasound (RUSI – Rehabilitative Ultrasound Imaging) to assess, non-invasively, how the abdominal and pelvic floor muscles behave.

  • It is performed externally, over the abdomen and/or perineum.
  • It is not a diagnostic prostate ultrasound, but a functional study aimed at seeing muscle activation, coordination, pressure and synergies during specific tasks.
  • It is especially useful after prostatectomy incontinence, erectile dysfunction or chronic pain, to individualise treatment.

We have more than 7 years of clinical experience using this tool in men.

5. Results and treatment plan

At the end, all findings are explained clearly and professionally. Based on what is found, an individualised treatment plan is set or therapeutic recommendations are given.
